TERMS AND CONDITIONS

A. TheResponsibility and Dutyof the Registered Company
(1) The operator who carries out the service shall possess the appropriate license issued by the
Government or have equivalent qualifications.
(2) The registered company shall be responsible for ensuring the safety of the operator from any danger
which can arise during operation.
(3) The registered company shall check the equipment to ensure the proper functioning of the
equipment before operation.
(4) The registered company, if necessary, shall have the equipment calibrated by authorized
organizations in order to maintain the required accuracy within the period prescribed in the
standard, and have the contents recorded and maintained.
(5) The registered company shall keep a copy of the service report submitted to this Society and
applicant for at least 5 years.
(6) The registered company shall notify this society immediately of any alterations of the condition at
the time of the approval, including changes of staff and of any entries on this certificate.

B. Confidentiality
The detail information acquired during the job shall not be disclosed or released to
third party without permission of the applicant or this Society.

C. Scope and Limitation
The extent of engagement is limited to the scope stated on this Certificate unless attached otherwise.

D. Renewal of theApproval
Renewal audit should be carried out within 3 months before the expiry date of Certificate. However, the
validity of new Certificate will be started from the next day of expiry date of previous Certificate.

E. Cancellation of Approval
The approval may be cancelled in cases:
(1) Where the service provided is improperly carried out or the results are improperly reported.
(2) Where the Society's surveyor has found major deviations in the operating system of the firm.
(3) Where the supplier fails to inform of any alteration to the operation system to this Society.
(4) Where occasional audit, when deemed necessary by this Society, has not been carried out.
(5) Where wilful acts or omissions are ascertained.
(6) Where the approval fees are not paid.
