Annexure I
Terms and Conditions of Issuance of the Certificate
The society reserves the right to invalidate this approval, under following conditions:
1. Where the service was improperly carried out or the results were improperly reported.
2. Where surveyor finds deficiencies in the approved service operating system of the supplier and appropriate
corrective action is not taken.
3. Where alterations have been made to the Quality System relevant to the service supplier certificate, without
written notification to IRS.
4. Where willful acts or omissions are ascertained.
5. Where any deliberate misrepresentation has been made by the service supplier.
6. Confidentiality (detail information acquired during the job) shall not be disclosed or released to third party
without permission of this society.
